Customer Notification re: Improper FM Approval Marking

Please note that you may be receiving Foamular® Extruded Polystyrene Insulation from the Rockford, IL manufacturing facility with unit wrapping that has been improperly marked with the approval of FM Approvals. The product you are receiving meets all ASTM C578 product and property specifications. However, due to a software error in our automated label printing system, the FM logo was inadvertently applied to unit labels for all products, but should have been applied only to FM Approved roof insulation products. The mislabeling appears on labels on four sides of the unit.

This error was discovered during a routine FM Approvals surveillance audit on December 2, 2010. The software error was corrected immediately, however, all products produced between September 20, 2010 and December 2, 2010 were potentially mislabeled. Since discovering the error, all product units have been shipped with the FM Approval mark obliterated, but prior to discovering the error, others may have shipped with the inappropriate logo visible.

The letter shown here is being provided to all distributors with each shipment to ensure that customers are aware of this issue.
